A hard fork changes a rule in a way old nodes reject, so everyone has to upgrade or the chain splits. The list has debated how to do one safely, whether it ever should, and what to make of the forks that happened anyway: Bitcoin Cash in 2017 and Bitcoin SV in 2018.
